Supervised Independent Living

Residential habilitation supported independently living services to approved or eligible waiver recipients. Forever caring also provide the services to anyone with a disability who is 18 years or older and desires to live on their own.

Areas of concentration for skills training include, but is not limited to personal care, home living, health and safety, financial guidance, and other various supports. 

This service is intended to serve anyone who is 3 years or older with a developmental disability. The objective for this program is to give performance of necessary daily living activities and respite care to relieve family members or caretakers of their responsibilities for a period of time.

Personal Care Attendant Services

Long Term Personal Care Services

​This service enables a person who requires care in a nursing facility or long-term facility to remain in their own home. This service is a Medicaid funded program designed to supplement a recipients family and community support.

Community Choice Waiver Services

EDA program is a service which is homebase for recipients 65 and older or disabled according to Medicaid or is a disability standards which would otherwise require institutional or nursing facility care.

Payment for these services are through Medicaid or state contract through Capital Area Human Services District (CAHSD), Louisiana Rehabilitation Services (LRS), and private pay. All billing entries are through Electronic Visit Verification  and LASRS.
